We use only the finest photographic material and inks in the production of these awards. All plaques are 100% unconditionally guaranteed and are free of defects when shipped. All shipments will be made by United Parcel Service.

To order a School Award Plaque, please download and print the included pdf files SAP Design Set up Page and Order Sheet (download worksheet). The Design Set Up Page will assist you in setting up and providing the necessary information to produce the custom plaque. The order sheet provides billing and contact information.

It will be necessary to send via regular mail the completed forms along with a photograph or CD with a digital image included to:

Please be sure to include your email address. A confirmation email will be sent upon receipt of your award information. After an award plaque design has been made, a low resolution sample will be sent for your approval along with the cost including shipping and handling.

Please return via email the sample along with your approval. Production will be two business days from receipt of your approval.

An invoice from School Award Plaques via Pay Pal will be sent with payment expected upon receipt. Authorized schools will have 30 days net with an approved purchase order.
